The plug-vs-patch distinction matters for safety. An external plug - the kind done in two minutes without removing the tire - fills the hole from the outside but does not seal the tire's inner liner. That liner is the airtight membrane on the inside of the tire carcass, and it is what actually holds the air. A plug that does not reach and seal the liner means air can still migrate into the tire's body through the unsealed liner hole.
